Fill Out Our Program Participation Form

I/We, the parent's/guardian of the above named child, give my/our approval to participate in The Hit Club LLC programs. I/We assume all risks and hazards incidental to such partitcipation.  I/We do hereby waive release, absolve, indemnify, and agree to hold harmless The Hit Club LLC and their Coaches, for any claim arising out of any injury to my/our child, which may included exposure to pandemics, like Covid-19, whether the result of negligence or for any other cause. In all cases related to claims, The Hit Club, LLC's liability is always limited to the extent and amount of any insurance coverage it may maintain. I/We, hereby authorize The Hit Club LLC and its assigns to utilize any and all photographs, pictures and or other likeness of me or anyone assigned guardianship to me, as they deem appropiate in its promotional materials.
